Female genital mutilation/cutting: knowledge,attitude and training of health professionals in inner city London (2013)

Objective: To assess the knowledge,attitude and training on female genital mutilation/cutting (FGM/C) amongst medical and midwifery professionals working in an area of high prevalence of the condition. Study Population: Health Care Professionals Findings: All respondents were aware of FGM/C but only 27.8% correctly identified the grade from a simple diagram. Three quarters (72.4% and 77.2% respectively) were aware of the complications of FGM/C and of the legislation in the United Kingdom. Of the respondents,13.9% agreed that a competent adult should be allowed to consent to FGM/C if requested but only 8.9% agreed that the procedure should be medicalised to reduce the associated morbidity. Less than 25% of respondents had received formal training in recognising or managing this condition. Although the majority of respondents were aware of FGM/C,their ability to identify the condition and its associated morbidity remain suboptimal; more training is recommended in larger cities with a higher prevalence of this condition Geographical coverage Region(s):Northern Europe Country(ies):United Kingdom Source

Female genital mutilation/cutting: Knowledge,practice and experiences of secondary schoolteachers in North Central Nigeria (2015)

Objective: To determine the experience of schoolteachers about FGM/C and their possible role in contributing to its eradication. Study Population: secondary schoolteachers from 18 secondary schools in Ilorin,North Central Nigeria Findings: More females than males were aware of FGM/C (205 v. 94; χ2 41.2; p=0.001); 180 women (69.8%) and 81 men (71.7%) wanted awareness and the implications of FGM/C to be taught in schools,while 46 women (17.8%) and 23 men (20.4%) had previously educated students about FGM/C. Also,109 (42.3%) of the female teachers had been mutilated (mean (standard deviation) age 4.76 (4.86) years),and 49 mutilations (45.0%) had been performed by traditional circumcisers. Of the teachers,44.0% of men and 24.5% of women had subjected their daughters to FGM/C (p=0.029),mostly for religious reasons. The men initiated the majority of their daughters’ mutilations,while the mothers-in-law were the main initiators among the women; 44 (17.0%) women and 23 (20.4%) men held the opinion that females should be circumcised,while the majority considered education and legislation to be the most important interventions to encourage its eradication. Predictors of the likelihood to support discontinuation of FGM/C included awareness of government policy about FGM/C and having a mutilated daughter.The study concluded that education,reorientation and motivation of teachers would position them as agents for eradicating FGM/C. The greater support towards the practice is found among traditionally practicing groups. A substantial proportion of men intend to have it performed on their daughters,although reporting a low involvement in the decision making process,with very few taking alone the final decision. Only a minority is aware of FGM/C health consequences,but those who understand its negative impact on the health and well-being of girls and women are quite willing to play a role in its prevention. In addition,the study aims to develop new nursing protocols and screening tools for FGM patients and their daughters Study Population: Nurses Findings: The study revealed three themes throughout the responses of our survey. Those three themes were procedure modifications,individualized therapeutic conversation,and cultural incompetency. Exam modifications included slower pelvic exams with the suggested use of a pediatric speculum and emla cream or lidocaine for the simpliest of procedures as well as the use of interpreters. Individualized therapeutic care includes but is not limited to,discussion of antenatal defibulaiton well before labor begins. Objective: To study the practice of female genital mutilation in Sharkia governorate Study Population: Mothers Findings: Results showed that 97.2% of girls in rural areas were circumcised,compared with 81.9% of the girls in urban areas. Of the various factors studied,the study found that mothers aged 40 years and older,those married before the age of 20 years,housewives,illiterate mothers,and those living in rural areas more significantly practiced daughter circumcision. Regression analysis revealed that the level of the fathers’ education and the area of residence were the most significant factors influencing this practice. Female Genital Mutilation: A growing challenge for midwives in the UK (2006)

Objective: To determine the need for antenatal defibulation and subsequent obstetric outcome of women with a history of FGM presenting to a London obstetric unit. Study Population: Women Findings: All women had undergone FGM and 67% required defibulation prior to delivery; 79% of women sustained either an episiotomy or tear. A significant number of women (38%) declined antenatal defibulation,preferring to wait until the onset of labour. This has implications for midwives and obstetricians who are not at present trained in the management of women with previous FGM presenting in labour. Although caesarean section and instrumental delivery rates were comparable to the hospital averages,all women — whether defibulated or not — had extremely high levels of perineal trauma.
